3 min readThis paper reviews the evidence for the role of sunlight and vitamin D in modulating immune response to SARS-CoV-2 and other respiratory viruses. Vitamin D, synthesized in the skin upon sunlight exposure, is a potent immunomodulator that regulates both innate and adaptive immunity. We synthesize epidemiological data showing higher COVID-19 mortality in populations at higher latitudes and in winter months, experimental evidence on vitamin D's effect on coronavirus replication, and clinical data on the relationship between vitamin D deficiency and COVID-19 severity. Our analysis supports the hypothesis that vitamin D supplementation and outdoor sunlight exposure may be effective tools for reducing COVID-19 morbidity and mortality.
The COVID-19 pandemic prompted urgent investigation into environmental and biological factors that modulate disease severity. Among the most compelling hypotheses to emerge early in the pandemic was the role of vitamin D — both because of its known immunological effects and because of striking geographical patterns in COVID-19 outcomes that tracked closely with latitude and seasonal variation in sunlight.
This work compiles and analyses the then-available evidence linking sunlight exposure, vitamin D status, and COVID-19 outcomes, and makes the case for public health messaging around outdoor activity and vitamin D supplementation as low-cost, safe interventions.
